State regulations require The Children's House to maintain a medical record on each child. A medical report on the Illinois Department of Public Health issued form, indicating the child has had a physical examination and that all immunizations are up to date (including TB test) must be turned in prior to your child's admission to our centers.
The completed certificate of health examination provided in your enrollment packet must be completed and include the Physician's stamp. The physical examination form must be dated within six months of your child's enrollment date. If your child is transferring from another licensed childcare center, the medical report may be used to enroll your child if it is less than one year old. The medical report shall be valid for two years, except that subsequent examinations for school age children shall be in accordance with the requirements of the Illinois School Code and the Child Health Examination Code, provided that copies of the examination are on file at the center. In the event of an accident or medical emergency, The Children's House shall contact parents and the doctor of your child should it be necessary. If neither can be reached and an emergency treatment is required, the child will be taken to the nearest hospital by ambulance. Your written authorization for The Children's House to contact the necessary medical professionals is a part of your enrollment package. For the protection of all children and staff, a child that shows any of the following symptoms should be kept at home. A.) temperatures over 100 degrees Should any of these symptoms occur while your child is at the center, he/she will be isolated for the safety of the other children and a parent or authorized person from your child's emergency form will be phoned to pick the sick child up. A written statement from the child's physician should be presented to the Site Director upon return. Please select another area below to find out more.
