The Children's House

Parents choosing The Children's House as their child care provider will be given several forms to complete so that we can best service your children. Included in this packet are identification forms, emergency forms, medical forms, a variety of consent forms, and tuition agreement and payment forms.

Please read carefully, sign and return all of these forms and agreements on your child's start date. Any forms that are incomplete could delay your child being able to start school. Parents/guardians are expected to abide by this agreement for the duration of your child's enrollment.
